Description: Based on review of four staff's records, the center failed to ensure that at least one staff member on duty that had a current DHO training. Evidence: 1. All staff members present on the day of the inspections did not have a current Daily Health inspection training. The training expired on 9/3/2016.

Plan of Correction: The staff will obtain a training for the DHO ASAP and will ensure at least one staff member is present at all times.

Description: Based on observation, the center failed to ensure that all requirements for when food is provided is met. Evidence: 1. The inspector found that the menu was not completed for yesterday and today for morning snack.

Plan of Correction: The staff will ensure that the menu is completed a week in advance.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the center failed to ensure that all lunches brought from home were dated with today's date.

Plan of Correction: The staff will ensure that all lunches are dated daily as they arrive.
