Comments:
An unannounced monitoring inspection was conducted today from 1:10pm-3:15pm. There were 110 children (infants-5yrs.) directly supervised by 20 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, using manipulatives, sight work activities, and sleeping. Teacher and child interactions were observed and viewed as positive. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not ensure all children's records contained required information. Evidence: 1) Child 1, 2, 4 are missing the address of two designated people to call in an emergency if a parent cannot be reached. 2) Child 1 and 5 are missing the work phone number of one or both of parents.

Plan of Correction: We will contact parents and get the missing information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, an area in the infant toddler room needed maintenance to keep the area in safe and operable condition. Evidence: In the Infant Room, a window sill had a small area of cracking paint. The area is located behind a rocking chair with can be accessible to children.

Plan of Correction: We will have it painted by the weekend.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: In room 4, the lock of a closet is broken and cleaning products marked keep out of the reach of children are kept in the aforementioned closet.

Plan of Correction: We will get the lock fixed and remove the cleaning products from the closet while we are waiting to get fixed.

Standard #: 22VAC40-185-290-3
Description: Based on observation, not all electrical outlets had protective covers on all unused outlets. Evidence: In room 10, there are two outlets above the sink area which did not have outlet covers. In the music room, there are two outlets uncovered.

Plan of Correction: Protective covers were placed into unused outlets on the date of inspection.

Standard #: 22VAC40-185-360-A
Description: Based on observation, a piece of equipment which is not age and stage appropriate is being utilized in the art room. Evidence: A paper cutter is being utilized in the art room for kindergarten students with the supervision of the art teacher.

Plan of Correction: Removed from the art room and placed in a locked room.
