Comments:
12/16 - Licensing Inspector arrived at 10:30 and departed at 11:45 AM. There were no children in care. An inspection of the physical plant and a record review were conducted. The sample size was six children's records. Violations were noted in the area of administration. 12/20 - Licensing Inspector arrived at 10:30 and departed at 11:15 AM to review the care of the children. There were 64 preschool aged children in care with eight staff. No violations were cited as a part of this inspection.

Standard #: 22VAC40-185-60-A
Description: Based upon review of six children?s records and staff interview, the facility has not ensured that each child?s record contains the names, addresses and telephone numbers for two designated persons to be contacted in an emergency when a parent cannot be reached. Evidence: 1. The record provided for child 4 contained a name and telephone number for one emergency contact, however a street address was not included. No information was provided for a second emergency contact person. 2. The record provided for child 5 contained the name, address and telephone number for only one designated person to be contacted in an emergency. 3. Staff 1 was unable to provide documentation of additional emergency contact persons for child 4 or child 5.

Plan of Correction: Family Involvement Specialists will ensure that all children's records are completed.

Standard #: 22VAC40-185-90--A
Description: Based upon review of six children?s records and staff interview, the facility has not ensured that each child?s record contains written parent agreements to 1.) pick up a child who becomes ill while in care and 2.) a statement that the parent will inform the center within 24 hours or the next business day after the child or a member of the immediate household has developed a reportable communicable disease. Evidence: 1. The record provided for child 2 did not contain a written parent agreement to pick up the child if the child becomes ill while in care. The file also did not contain the parent statement that they will inform the center within 24 hours or the next business day if the child or a household member develops a reportable communicable disease. 2. Staff 1 was unable to produce the above written agreements for child 2.

Plan of Correction: Family Involvement Specialists will ensure that all children's records are completed.
