An unannounced monitoring inspection was conducted on December 21, 2016. There were 36 children present with 4 staff members supervising the children's activities. The ages of the children ranged from 4 years to 12 years. The children were observed in the gym during free play and snack time. The children viewed a movie during the last hour of the program in the front child care room. During the morning program, the children participated in a swimming activity, an organized game, and crafts. For lunch, the children were treated with special foods prepared by parents and staff. The children were visited by Santa and had pictures taken. The children made an ornament to give their parents. A sample of 4 children's records was reviewed. Documentation of the Center's annual fire and health inspections was current. Documentation of the practiced emergency drills and emergency numbers were posted in the center. As reported by the Coordinator, there are currently no children's medications in the Center. Findings of the inspection were reviewed with the Center Coordinator. Standard #: 22VAC40-185-130-C
Description: Based on a sample review of children's records, the Center failed to obtain documentation of additional immunizations once between each child's fourth and sixth birthdays. Evidence: One of four reviewed records (child #4) did not contain an updated immunization record. The child's birth year is 2007, the immunization record available for review was dated 2010.

Plan of Correction: The School Age Coordinator informed the parent by e-mail at the time of the inspection. Documentation of the school entrance physical/immunization record will be obtained and filed in the child's record.

Standard #: 22VAC40-185-140-B
Description: Based on a sample review of children's records, the Center failed to ensure documentation of the physical examination accepted at the time of attendance was completed within the required time period based on the age of the child: within 12 months prior to attendance for children two years of age through five years of age. Evidence: One of four reviewed records did not have documentation of a current examination based on the time period of attendance. The child (#4) was enrolled in the program at 5 years of age, the physical examination was completed at 3 years of age.

Plan of Correction: The School Age Coordinator notified the parent at the time of the inspection and has requested documentation of the child's school entrance physical examination. Documentation will be maintained in the child's record.

Standard #: 22VAC40-185-540-B
Description: Based on direct observations, the Center failed to ensure the first aid kit was accessible to staff but not to children. Evidence: The LI observed the first aid kit located on a box which was placed on the gym floor next to a wall. Children were present in the gym and the kit was located within their reach. Upon notice, staff removed the kit and placed it in the program's supply closet.

Plan of Correction: Staff will be reminded to keep the first aid kit out of reach of the children.

Standard #: 22VAC40-185-550-C
Description: Based on direct observations, the Center failed to post the emergency evacuation and shelter-in-place procedures in a location conspicuous to staff and children on each floor of each building. The map identifying the emergency routes was posted on the information board located in the gym. This is a repeat violation.

Plan of Correction: The School-age Child Care Coordinator will print the procedures and ensure that the document is posted on the information board with the map and on the second floor of the building. Staff will be instructed to monitor the areas to ensure the procedures are posted.
