An unannounced monitoring inspection was conducted with regional specialist and assistant director. There were 64 children present, ranging in age from 5 months to six years old. The children were engaged in a variety of activities including free play, art projects, arriving from school, and afternoon snack. Snack served today: pretzels and cheese. Three emergency medications for three children were reviewed - please see Supplemental Summary for missing items. Four staff are certified in Medication Administration Training. Two staff files and two children's files were reviewed and are in compliance. The last health inspection was conducted 8/16/2016. The last fire inspection was conducted 10/13/2016. The artwork in the pre-Kindergarten classroom shows excellent programming. Standard #: 22VAC40-185-80-A Description: Based on a review of the attendance records, the facility failed to ensure all children were included on the daily written attendance record. Evidence: in Preschool 2, there were 17 children present; however, only 13 children were included on the daily written attendance record. Plan of Correction: All staff will be retrained on attendance and accountability. All children will remain in proper assigned classrooms.
Standard #: 22VAC40-185-350-F Description: Based on observation and interview with staff, the center failed to keep children within their regular group, by casually reassigning children for reasons other than administrative necessity, and disrupting the children's schedule and attachment to their regular group. Evidence: three children were moved from Pre-K to Preschool 2, an older infant was moved to the younger infant group, one toddler was moved to the older infants room, and another toddler was moved to the twos room. Plan of Correction: All children will remain in assigned classrooms. Management will be retrained on proper transitions.
Standard #: 22VAC40-185-500-B Description: Based on observation, the facility did not contain soiled linens in a lidded, leakproof container. Evidence: there was a bag of soiled linens on the sink counter. Plan of Correction: When employees leave for the day, all soiled linens will remain in soiled container with lid and taken to laundry.
Standard #: 22VAC40-185-510-A Description: Based on review of medications, the facility failed to have a completed long term medication authorization for each medication on site. Evidence: The long term medication form (MAT model form) was missing the first page of the authorization. The first page states the child's name, the name of the medication, the dosage amount, and the time or times to be given. Plan of Correction: Parent faxed the first page of the authorization to the center at the conclusion of the inspection. 11/22/2016
Standard #: 22VAC40-185-550-D Description: Based on review of the emergency evacuation drill log, the facility failed to document a practiced drill each month as required. Evidence: there was no drill documented for October 2016. Plan of Correction: Facility will document all monthly drills and include two management signatures.
