Comments:
An unannounced monitoring inspection was completed on January 6, 2017. The purpose of this visit was to measure the facility?s compliance with most but not all of, the licensing standards. The following ratios were observed: No infants in care on this date. Toddlers- 4 children/1 staff Twos- 5 children/1 staff Preschool-11 children/2 staff The toddlers were playing with a variety of toys. The twos were playing with play-doh. The preschool children were watching The Jungle Book. The children were served a lunch of a macaroni and cheese, peas, pineapple and milk. Per the director, the facility does not administer medication. The last evacuation drill was conducted 12/20/16. The most recent shelter in place drill was conducted 7/6/16. Two staff records and four children?s records were reviewed. Approximate time on site: 10:08am to 12:30pm

Standard #: 22VAC40-185-580-C
Description: Based on a review of the facility van, the facility did not ensure that all of the required information was in the van. EVIDENCE: The facility did not have a document which included the required emergency numbers and emergency shelter information in the vehicle.

Plan of Correction: Corrected on site.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on a review of a sample of staff records, the facility did not ensure that a sworn statement was updated within 3 years. EVIDENCE: The record of staff person #2 contained a sworn statement dated 12/18/13. The sworn statement had not been updated within 3 years.
