Comments:
An unannounced renewal inspection was conducted on January 18, 2017, between the hours of approximately 10:10 a.m. and 4:00 p.m. Eighteen (18) children were enrolled and in the care of seven staff members, in four classrooms, during the on site inspection. One additional staff member was present and assisted, as needed. Four children's records and eight staff records were reviewed and/or updated. One medication and one topical ointment, as well as authorization forms, were reviewed. Posted information (license, licensing inspection documentation, emergency evacuation diagram and plan, emergency telephone numbers, menu and classroom schedules), safety equipment (first aid kit, flashlight and radio) and available documents (emergency evacuation drill and shelter-in-place drill records, daily infant forms, attendance record injury reports and insurance) were reviewed/discussed. The daily schedule accurately reflected the activities provided. Lunch and snack were provided by the children's parents and breakfast was provided by the facility. Program observation was completed. Activities observed on this date included: indoor play with classroom materials/toys; stories; restroom and handwashing procedures; lunch; infant care (feeding, sleeping, play and diapering); naptime and pick-up departure of children. Staff/child interactions were observed to be positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Exceptions to program compliance were noted on the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on review and/or update of four children's records and discussion with staff #1 on January 18, 2017, the program failed to obtain documentation of viewing proof of one child's identity and age. Evidence: 1. Review of child B's record, on this date, determined that verification of age/identity was not included in the record. 2. Staff #1 confirmed the lack of verification of age/identity for child B.

Plan of Correction: The child's parent was immediately contacted and she is bringing the documentation tomorrow.

Standard #: 22VAC40-185-210-A-4
Description: Based on review of two staff records and discussion with staff #1 on January 18, 2017, the program failed to ensure that one staff member had six months of programmatic experience prior to being hired in the position of program leader and one staff member had completed 24 hours of training within one month after being promoted (to meet the program leader qualifications). Evidence: 1. Review of staff #7's record, on this date, determined that the employee had only 3 months of programmatic experience (hire date 10/24/2016) and staff #3 had completed only 16 1/2 hours of the required 24 hours of program leader training. The staff member was hired in the position of "program leader" on 8/15/2016. 2. Staff #1 confirmed the hours of completed training, the staff members' experience and the staff members' positions.

Plan of Correction: Staff #3 will have the training completed by the end of next week. Additional experience will be obtained from staff #7 and documented. If needed, additional experience will be obtained.

Standard #: 22VAC40-185-340-D
Description: Based on observation and discussion with staff #1 on January 18, 2017, the facility failed to ensure that at least one staff member met the qualifications of a program leader or program director in each grouping of children. Evidence: 1. Classroom observation, review of staff records and discussion with staff #1 determined that the staff members supervising classrooms #1 and #2, on this date, did not meet the program leader/program director qualifications. 2. Review of staff records established that staff #3, supervising classroom #1, had completed only 16 1/2 of the required 24 hours of program leader training. Staff #7, supervising classroom #2, lacked 3 months of programmatic experience. 3. Staff # 1 verified the training hours completed by staff #3 and the experience of staff #7.

Plan of Correction: Staff #3 will complete the training by the end of next week. In the future, staff members requiring 24 hours of training will complete it within the week of orientation. Past experience will be documented for the staff member needing 6 months of experience.
