Comments:
An unannounced monitoring visit and facility tour was conducted from 11:18am-12:00pm. At the time of inspectors' arrival there were 15 children in care with 2 staff members. Children were observed partcipating in free play, instructional time in the classroom, transitioning between activities and hand washing. A sample of 3 children's records and 3 staff records were reviewed. Areas of noncompliance are detailed in the violation notice and were discussed with the Director during the exit meeting at the conclusion of this inspection.

Standard #: 22VAC40-185-340-F
Description: Based on observation it was determined the center did not ensure that all children under 10 years of age are within actual sight and sound supervision of staff. Evidence: At the time of inspectors arrival 11:18 am there were 3 children between the ages of 3-5 years in a classroom off of the main class with the door closed. There were 2 staff present in the main classroom with 12 children but no staff member was in the smaller classroom with the three children. The staff went to the smaller classroom and opened the door at 11:21am.

Plan of Correction: Director will retrain all staff on sight and sound supervision procedures.

Standard #: 22VAC40-185-550-E
Description: Based on record review it was determined the center did not maintain a record of the dates of the practice drills for one year. Evidence: There was no drill documented for the month of December. Staff reported that the drill was done early in the month of December but they did not document it.

Plan of Correction: Director will document all drills immediately following the drill.
