Standard #: 22VAC40-185-340-C
Description: Based on observation on 1/11/2017 the center did not ensure that when one or more children are present there is one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age. EVIDENCE: 1. From 8:10a.m. to 8:23a.m. there were 13 children present with one staff, Staff #1. The children ranged in age from infants to school age.

Plan of Correction: The director has reiterated through a staff meeting and amendment to Employee Handbook, that at no time a staff to be alone with a child in the building. If one staff is alone & the 2nd staff person is late or anticipates an absence, they must first notify the director immediately. However, If a child comes & the staff is alone, the parent will have to stay with the one staff until a 2nd staff arrives or the parent must take their child with them. The parent will not be able to leave the child with one staff. NO EXCEPTIONS. In addition, The Parent's handbook has been amended to show this change.

EVIDENCE: From 8:10 am to 8:23 am, the ratio for the mixed age grouping of children at the center was 11children to one staff. From 8:23am to 8:27am the ratio was 13 children to one staff. The children in the grouping ranged in age from infants to school age children.
