22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted beginning at 8:20 a.m. and ending at 11:30 a.m. with the assistant director. There were 79 children in care with 10 staff ranging in age from three months to ten years. School agers were observed having quiet time before the bus arrived. The JK class was observed during circle time. Three year olds were observed during morning centers that included imagination play, home living and legos. Two year olds were observed during circle time. Diapering procedures were observed in the two year old classroom and the older infant rooms. Toddlers and older infants were observed having free play with toys and sensory bottles. Infants were observed having playtime on the floor and being fed. Five children's records and four staff records were reviewed. Please have the plan of correction and date to be corrected by returned by January 31, 2017. Standard #: 22VAC40-185-60-A
Description: Based on review of documentation, not all records were complete. Evidence: Child #1, enrolled 5/22/2014, did not have full addresses for two emergency contacts.

Plan of Correction: This was corrected during inspection. The parent was contacted and came in during the inspection to correct the missing information.

Standard #: 22VAC40-185-350-E-5
Description: Based on observation, the center failed to maintain the required staff to children ratio. Evidence: In the school-age room, Staff A was observed alone with twenty-one children ranging in age from five to ten years old. The required ratio for school age children ages five to eight years old is one staff member per eighteen children.

Plan of Correction: Staff members have been counselled to call management prior to leaving the classroom in order to avoid being out of ratio. Communication strategies have been implemented to ensure that if a teacher needs to leave a classroom he/she will tell us how many children are in the classroom before leaving. A staff member will step into that classroom before that teacher steps out so ratio are always maintained. Another part time staff have been hired to work in the morning. She started on Friday 1/27/2017.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of documentation, the center failed to obtain central registry findings within 30 days of employment. Evidence: Staff B, start date 9/16/2016, central registry background check was dated 12/5/2016.

Plan of Correction: Going forward we will save the copy of the central registry copy with the mailed date it was sent and contact central registry on the 30th day by telephone to show record that we have not yet received the background check. The call will be documented on the copy of the paperwork to include the date and the time of call along with the name of the individual we spoke with.
