Each classroom attendance must accurately reflect the children in care at all times, including transitioning children.
An unannounced monitoring inspection was conducted today. There were 63 children in care, with a total of 11 staff members assigned to their direct supervision. The children were observed finger painting, matching shapes, practicing their letter/sound recognition skills and spelling, making groundhog day books, and eating lunch. The children were served grilled cheese sandwiches, tomato soup, pineapple, and milk for lunch today. This inspection was completed between approximately 10:15 am and 12:20 pm. Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #4 does not include the home phone number and each parent's work phone number. Plan of Correction: I will ask the parent for the missing information.
Standard #: 22VAC40-185-270-A Description: Based on observations, it was determined that various areas of the school have not been maintained in a clean, safe, and operable condition. Evidence: 1. In the Infant's classroom, the drawer beneath the handwashing sink has come into disrepair leaving a large area of exposed particle board. 2. In many of the children's bathrooms, the tiles are heavily stained. 3. On the 2's playground, a broken/sharp window screen has been placed on top of the children's play table. 4. On the Preschool playground, the heavy plastic covering that coats the metal ladder steps has come into disrepair leaving three areas of exposed metal that is sharp and rusted. 5. The exit door leading from the 2's classroom is broken and slams shut. This door is very heavy and could cause injury to a child if the door were to slam shut on a child's body part. Plan of Correction: 1. I will ask maintenance to repair or replace the handwashing sink cabinet. 2. I have already requested for the bathroom tiles to be replaced. I will follow up. 3. The window screen will be discarded today. 4. I will report the damaged ladder steps to maintenance. 5. I will request an emergency repair for the classroom exit door.
Standard #: 22VAC40-185-280-B Description: Based on observations, it was determined that one potentially hazardous substance was not locked. Evidence: In the 3's classroom, the spray bottle of table sanitizing solution was stored on the floor inside an unlocked closet. Plan of Correction: The cleaner was moved to a locking cabinet during the inspection.
Standard #: 22VAC40-185-330-B Description: Based on observations, it was determined that various areas of the playgrounds do not have enough resilient surfacing. Evidence: There is not enough mulch beneath each of the slides on both playgrounds. In each of these areas, the mulch has worn down to dirt. Plan of Correction: I will request additional mulch to be installed.
Standard #: 22VAC40-185-510-C Description: Based on a review of children's medications, it was determined that one authorization form indicates an expired date. Evidence: The medication authorization form for one child's long-term prescription medication indicates an expiration date of 1/21/17. Plan of Correction: The parent just returned that form. It appears the doctor mistakenly wrote 2017 instead of 2018. I will ask the parent to have the form corrected.
Standard #: 22VAC40-185-570-E Description: Based on observations, it was determined that one bottle of breast milk was not refrigerated as required. Evidence: In the Older Infant's classroom, one child's bottle of breast milk was left out on the counter after the child had finished drinking from the bottle. Plan of Correction: The bottle was emptied during the inspection.
