An unannounced monitoring inspection was conducted in the Center on February 9, 2017. Upon arrival, the LI observed a staff member and one child present. After school, 6 additional school-age children arrived to the Center. As reported, nine children are currently enrolled in the Center. Documentation of the Center's annual fire and health inspections was current. A sample of 7 records was reviewed: 4 children's, one new staff and 2 staff whom were previously employed. Information in children's records was also reviewed to verify compliance from the inspection report dated 08/18/2016. A discussion was held with both staff members regarding staff schedules and to always ensure at least 2 staff are present in the Center whenever a child is in care. Standard #: 22VAC40-185-130-A
Description: Based on a review of children's records, the Center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the Center. Evidence: Child #5 was present at the time of the inspection, the record did not contain documentation of an immunization record. Staff reported that the parent requested the information at the time the child was picked up and the information is to be faxed to the center.

Plan of Correction: Staff will follow-up to ensure the information is obtained and filed in the child's record.

Standard #: 22VAC40-185-60-A
Description: Based on a sample review of 3 children's records, the Center failed to maintain a complete record for each child. Evidence: 1. The record of child #1 did not indicate the last date of attendance. 2. The record of child #4 did not contain the name and phone number of the child's physician. 3. The record of child #3 did not address chronic physical problems and pertinent developmental information, if any

Plan of Correction: The parents will be requested to provide the information to complete each child's record.

Standard #: 22VAC40-185-70-A
Description: Based on staff report and review of 3 records, the Center failed to maintain a complete record for each staff person. Evidence: 1. The record of staff #1 noted date of hire as 01/26/17, position Director. The following information was not available for review: verification of age requirement, documentation of 2 or more references, written information to demonstrate that the individual possesses the education, orientation training, staff development, certifications and experience required by the job position. 2. The record of staff #3 did not indicate the date of separation from employment.

Plan of Correction: Documentation will be provided to the Center and kept on file.

Standard #: 22VAC40-185-340-C
Description: During the stated hours of operation, the Licensee failed to ensure when one or more children are present, there is always one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. Evidence: Upon arrival in the Center at approximately 1:15pm, the LI observed only one staff member present with a four year old child in care. The attendance sheet noted the child arrived in the center at 6:49am. At approximately 2:30 PM, 3 additional school-age children arrived at the Center after school. At this time, 4 children and 1 staff member was present. Staff member #1 arrived back at the Center at 3:15 PM. As reported, Staff 1 had worked this morning in the Center from approximately 10:00am to noon.

Plan of Correction: The Center will always have at least 2 staff persons present in the center whenever a child is in care. Staff 1 will be scheduled to work full time until other staff is employed. The center is currently interviewing for additional staff.
