Standard #: 22VAC40-185-510-C
Description: Based on observation, medication kept on site did not have the medication form completely filled out. Evidence: A medication that is intended for long term housing at the center, did not have a physicians signature. Without physician's signature, medication can not stay at a site longer than 10 days.

Plan of Correction: I will give the parents a short term medication form and will make sure they have the doctor complete the new form with the information regarding when the medication is to be administered.

Standard #: 22VAC40-185-510-G
