63.2 General Provisions.

Comments:
An unannounced monitoring and other investigation was conducted on February 16th 2017 between 11:15am and 1:15pm. There were 65 children in the direct care of 12 staff members. Children were observed playing on the playground, playing the indoor activity space, washing hands, eating lunch (chicken, bread, milk, fruit, and vegetables), and singing songs. A sampling of children's records, staffing records, the physical space, and supervision were observed and inspected. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center was not maintained in a clean, safe, and operable condition. Evidence: In a toddler room, a closet door is coming off of it's hinge. In the 2's classroom, there were holes in the dry wall that could be pulled apart by a child.

Plan of Correction: Will fix the drywall and a parent volunteer is repairing the closet door.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous materials were not kept in a locked area. Evidence: In the preschool room a bleach bottle was in an unlocked closet.

Plan of Correction: We will replace the lock system in Rising Stars.

Standard #: 22VAC40-185-340-F
Description: Based on interview, sight and sound supervision was not in compliance. Evidence: On February 8th, the director called the licensing inspector explaining that a child had been left in the hallway for approximately 2-3 minutes. The director explained that the child was brought, by a parent, to the classroom and then to the director.

Plan of Correction: We wrote up the teachers and reviewed the center supervision policy. We are revising the playground sign in sheet.

Standard #: 22VAC40-185-430-L
Description: Based on observation, an infant walker was in the infant classroom. According to state standards, infant walkers are not to be used.

Plan of Correction: We took away the walker and will talk to the teachers about the use of infant walkers.
