- Technical Assistance:
1. Reviewed requirement for staff to sit with children at lunch. 2. Cracked plastic toys/equipment are considered to be a hazard and should be removed. 3. Tile in the school-age classroom is raised and cracked and the sink in the cabinet in the 3's classroom is starting to crack and deteriorate, and should be replaced soon before becoming a hazards. 4. Reviewed Kindercare medication forms.
Conducted an unannounced monitoring inspection 9:55am-1:00pm. Observed 66 children + 13 direct-care staff. Ratios and supervision were in compliance. Staff were leading activities and playing with the children in all groups. Activities included: writing alphabet letters, art, blocks, circle time, story time, outdoor active play, lunch, and naps. The infants were being held, fed, and having play time on the floor. Lunch served: ground chicken, tortillas, cheese, zucchini, pineapple, and milk. Building renovations have included new flooring in the common areas, painted walls and window ledges in classrooms, and new furniture/equipment throughout the building. A new climber has been installed on the playground, and wood mulch was in compliance today. Medication management was in compliance. Required documents were posted. Record keeping violations found. Standard #: 22VAC40-185-130-C Description: Based on records reviewed, it was determined that 1 of 4 children's records did not include immunizations between the 4th - 6th birthdays. Evidence: Child #3 - Date of birth 2/24/11, and immunizations dated May 2013. Plan of Correction: We have contacted the parent today, and file will be updated today.
Standard #: 22VAC40-185-270-A Description: Based on observations made, it was determined that areas and equipment of the center was not maintained in a clean, safe, and operable condition. Evidence: 1. Carpet - There was black hard ground in dirt/grime on carpeting in multiple classrooms. Areas most dirty were along the edges of where the carpet meets the tile. 2. Equipment - One riding tryke on the outdoor playground did not have rubber handle grips, which exposed the sharp metal handle ends. Plan of Correction: 1. We will work with the facilities department for replacement. They will be contacted today. 2. The tryke will be removed from the playground today.
Standard #: 22VAC40-185-290-3 Description: Based on observations made, it was determined that electrical outlets did not have protective covers. Evidence: A power strip in the school-age room, where preschool-age children were observed in activities today, had 5 outlets without protective covers; and a power strip in the infant room had 3 outlets without protective covers. Plan of Correction: Outlets covers were placed in all outlets today.
Standard #: 22VAC40-185-550-M Description: Based on records reviewed, it was determined that 3 of 5 written injury reports did not include required information. Evidence: 2 reports did not document how the parent was notified, and 3 reports did not document the time the parent was notified. Plan of Correction: We talked with staff today and discussed missing information.
Standard #: 22VAC40-191-60-C-2 Description: Based on records reviewed, it was determined that 1 of 3 staff records did not include a Central Registry Check (CPS) within first 30 days of employment, and the staff person has not been denied employment. Evidence: Staff #8 - Date of hire was 10/5/15, and there is not a completed CPS check on-file. Plan of Correction: We will call the registry today and resend a new form by Friday.
