Technical Assistance:
1. Discussed preventing access to unused children's areas and accessibility to staff file information.

Comments:
An unannounced renewal inspection was conducted today from 9:00am-12:45pm.The physical plant, 3 staff records, 6 children's records, 7 medications, evacuation drills, injury reports and policies were inspected. Children were observed participating in group play, circle time, playing outside, and eating snack. Diapering and hand washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-80-A
Description: Based on record review, the center did not keep a written record of children in attendance each day. Evidence: 1. Classroom 117 and 112 did not have a written for the day.

Plan of Correction: We will remind staff to take attendance daily.

Standard #: 22VAC40-185-260-A
Description: The center did not have a current annual fire inspection. Evidence: 1. The fire inspection available expired on 12/31/16

Plan of Correction: We will call and schedule the inspection.

Standard #: 22VAC40-185-340-F
Description: Based on direct observation a child was not keep within actual sight and sound supervision of a staff member. Evidence: 1. A preschool child was observed standing alone, in the hallway. 2. Staff A arrived minutes later that the child was waiting for her, while she used the rest room.

Plan of Correction: We will remind the teacher about sight and sound supervision.

Standard #: 22VAC40-185-510-J
Description: Based on observation, 2 epi-pens were not kept in a locked place an inaccessible to children. Evidence: 1. In classroom 117, 2 epi-pens were observed in a fanny pack, on a shelf.

Plan of Correction: We will remind the staff to keep the epi-pens on their person.

Standard #: 22VAC40-185-550-M
Description: Based on record review, 2 of 8 injury reports were incomplete. Evidence: 1. The injury reports for 11/28/16 @ 2:30p and 9/14/16 @ 11:55a did not document the time the parent was notified.

Plan of Correction: We will remind staff about the required injury report information.
