Comments:
An unannounced monitoring inspection was conducted on March 7, 2017 during the after care portion of the school age program. During the inspection there were 11 children present at arrival to the facility with three staff members. There are no medications present or being administered by the program staff. The licensing inspector observed the children doing homework and playing with various tabletop activities to include Legos, blocks, toy animals, arts and crafts and board games. The children also participated in a cooking activity which consisted making of St. Patrick's Day pancakes. Two children's records were reviewed. The licensing inspector observed injury report documentation, emergency and non-emergency supplies and documentation, indoor and outdoor areas, required postings, supervision, and health and safety.

Standard #: 22VAC40-185-70-A
Description: Based on review, the licensee did not ensure that required information to be kept at the center was available for each staff. Evidence: During the inspection conducted on March 7, 2017, the following information was not available at the center for Staff #2: name, address, verification of age requirement, job title, and date of employment or volunteering; and name, address and telephone number of a person to be notified in an emergency.

Plan of Correction: The staff member filled out a form with all required information during the inspection.

Standard #: 22VAC40-185-260-A
Description: Based on review of documentation and inspection of the facility, the center did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on March 7, 2017, the most recent fire inspection report for the facility was more than a year old and was dated 12/15/15.

Plan of Correction: The Area Manager is aware of the fire inspection report and the results of the inspection are pending.

Standard #: 22VAC40-185-280-B
Description: Based on observation and inspection of the facility, the staff members did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection conducted on March 7, 2017, the licensing inspector observed a bottle of disinfectant spray being taken from an open cubby in the cafeteria. The bottle was labeled keep out of reach of children and caution.

Plan of Correction: Site director will continue to monitor and keep the bottle in a locked cabinet out of children's reach.
