Comments:
An unannounced renewal inspection was conducted today from 9:45am-12:00pm. There were 82 children directly supervised by 11 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, engaged with manipulatives, painting, playing outside, and eating snack. Snack served consisted of pears, crackers, cheese stick and water. Hand-washing procedures were also observed. There was an abundant supply of materials for the children. Interactions between children and staff observed were viewed as positive. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, follow up tuberculosis screenings were not repeated two years from the date of the previous tuberculosis screening. Evidence: Staff 1 has a tuberculosis screening valid until 10/1/2016.

Plan of Correction: Staff 1 is scheduled to acquire a follow up tuberculosis screening today March 28, 2017.
