Comments:
The licensing inspectors conducted an unannounced subsidy health and safety inspection from 9:50am - 1:00pm. There were thirty nine children in care: 5 infants with 2 staff, 8 toddlers with 2 staff, 12 twos and threes with 3 staff, and 14 preschoolers with 2 staff. Two additional staff were also present. The children were observed at snack time, playing in the centers, painting, and playing on the playground. Eight staff and three children's records were reviewed. Violations:
Standard #: VENDSUB-000-012-C
Description: Based on record reviews, the center failed to have all staff complete their Virginia pre-service training. Evidence: 1. The inspector reviewed eight staff records. One of eight records reviewed (staff record #3) did not have documentation that they had completed their pre-service training. The staff member was employed prior to the effective date of the vendor agreement.

Plan of Correction: The director said that staff #3 will complete the pre-service training and provide a certificate for the record.

Standard #: VENDSUB-000-012-E-1
Description: Based on record reviews, the center failed to have all staff trained in cardiopulmonary resuscitation (CPR). Evidence: 1. The inspector reviewed eight staff records. Two of eight records reviewed (staff records #2 and #7) did not have documentation of current CPR certification.

Plan of Correction: The director will contact the instructor and schedule a CPR class for all staff who do not have current certification.

Standard #: VENDSUB-000-012-E-2
Description: Based on record reviews, the center failed to have all staff trained in first aid. Evidence: 1. The inspector reviewed eight staff records. Two of eight records reviewed (staff record #2 and #7) did not have documentation of first aid training.

Plan of Correction: The director will contact the instructor and schedule a first aid class for all staff who do not have current training.

Standard #: VENDSUB-000-031-B-3
Description: Based on review of the center's emergency preparedness plan, the center failed to include all components of the lockdown procedure. Evidence: 1. The inspector reviewed the center's emergency preparedness plan and all components of the lockdown procedure were not included in the plan.

Plan of Correction: The director will include all components of the lockdown procedure in the emergency preparedness plan and will review with staff.
