Comments:
Today's visit was a renewal inspection. The inspection began at 10:00am and concluded at 2:30pm. There were fourteen children present with four staff members. During the inspection two out of nine staff records were reviewed, three out of twenty-five children's records were reviewed, all four records for board officers were viewed, the playground was viewed, all three classrooms were viewed along with activities, the school age room was viewed, emergency supplies were viewed, and some policies and procedures were reviewed. Your renewal application has been received in the office. Once the application has been deemed complete, recommendation regarding your license can be made. Violations:
Standard #: 22VAC40-185-330-B
Description: Based on the inspector's observations on the playground used by the preschool children, the program failed to ensure a piece of climbing equipment had the required fall/trajectory zone. Evidence: The inspector observed a piece of climbing equipment that has a fall/trajectory zone of four feet for two sides of the equipment. The equipment requires a six foot fall/trajectory zone of resilient surfacing that is not interrupted by borders or other pieces of equipment.

Plan of Correction: The head of maintance stated he would have the fall zone corrected within one month of today's inspection.

Standard #: 22VAC40-185-550-D
Description: Based on the inspector's review of the log used to document monthly emergency evacuation drills, the program failed to practice a drill for the month of March. Evidence: The log to document fire drills stated there was no drill for the month of March.

Plan of Correction: Monthly fire drills will conducted and documented.

Standard #: 22VAC40-191-60-C-1
Description: Based on the inspector's review of all four board officers, the program failed to ensure the results of the criminal record background check was received within thirty days of the start of taking the position of a board officer.

Evidence: The record for the current Treasurer did not include the results of the criminal background check. Staff stated the officer has held his position since 7/16.

Plan of Correction: The compliance officer stated the background check had been received and could not be located during the inspection. A new request will be made if the CRC cannot be located.

Standard #: 22VAC40-191-60-C-2
Description: Based on the inspector's review of all four board officers, the program failed to ensure the results of the search of the central registry background check was received within thirty days of the start of taking the position of a board officer.

Evidence: The record for the current Treasurer did not include the results of the search of the central registry. Staff stated the officer has held his position since 7/16.

Plan of Correction: The compliance officer stated the background check had been received and could not be located during the inspection. A new request will be made if the CRC cannot be located.
