04/19/2017 Arrived 2:30 pm Departed 4:40pm An unannounced subsidy inspection was conducted today. Thirty-seven children were in care with three staff. Description: Based on the staff records and staff interviews, the facility failed to require first aid training for all staff within 90 days of signing the vendor agreement. Evidence: 1. Staff #1 and 2 have not completed first aid and CPR training.

Plan of Correction: The first aid class is scheduled for April 29, 2017.

Description: Based on a review of documents and staff interviews, the facility failed to have written evacuation plans. Evidence: 1. There are no written emergency evacuation plans.
