22VAC40-191 Background Checks (22VAC40-191)
The Licensing Inspector for the YMCA Afterschool Care @ Mt. Olivet conducted an unannounced monitor inspection on 4/25/2017. Upon arrival at 2:40 PM there were a total of thirty (30) school age children under the direct care & supervision of three (3) staff members. Three (3) children records, two (2) recently hired staff records, one (1) additional staff record & two (2) medication records were reviewed during today?s inspection. LI Anderson observed children during arrival/departure procedures, bathroom/hand washing procedures, snack/meal served, group activities, self-initiated activities and homework/quiet time. The exit interview including reviewing the Risk Assessment and areas of non-compliance found during today?s inspection on 1/17/2017 were reviewed with the Site Director on 1/17/2017. Inspection concluded at 5:30 PM. Standard #: 22VAC40-185-540-C Description: Based on inspection, the Licensee failed to have a required supplies for the first aid kit. EVIDENCE: The first aid kit did not have a digital thermometer Plan of Correction: Will fix tomorrow. Additional thermometers will be purchased

Standard #: 22VAC40-191-60-C-1 Description: Based on record review, the Licensee failed to ensure new employees complete Background Check (Criminal History Record) within 30 day of employment. EVIDENCE: The Background Check (CRC) for Staff Member #1 was not completed within 30 days of hire. Staff #1 date of hire was 12/06/2016 & the CRC was stamped received 2/07/2017 with a completion date 2/09/2017. Plan of Correction: Hiring process has been changed. Background checks will be submitted within the first 7 days of hire.

EVIDENCE: The Background Check (CPS) for Staff Member #1 was not completed within 30 days of hire. Staff #1 date of hire was 12/06/2016 & the CPS was stamped received 1/20/2017 with a completion date 2/02/2017. Plan of Correction: Hiring process has been changed. Background checks will be submitted within the first 7 days of hire.
