Today's visit was a renewal inspection. The inspection began at 3:00pm and concluded at 4:45pm. A second inspection day took place on 4/25/2017 in order to review staff records at the agency's main location. The subsidy health and safety inspection also took place during today's visit. During the time of the inspection snack time was observed, hand washing was observed, three out of forty children's records were viewed, emergency supplies were viewed, departure of some children was observed, some policies and procedures were reviewed, all four staff records were viewed on 4/25, and one medication with authorization was viewed. Outdoor play was not observed due to rain. Your renewal application has not been received in the licensing office. Once the application and today's inspection has been deemed complete a recommendation regarding your license can be made. Standard #: VENDSUB-000-005-B Description: Based on the inspector's review of attendance logs used the day of the inspection, the center failed to ensure a written record (hard copy) of daily attendance includes documentation of the children's departure. Evidence: The program uses an attendance log to capture the arrival of the children as it occurs and does not document depature of the children as it occurs Plan of Correction: Attendance logs will be altered to include "in and out" and will be documented as arrival and departure occurs.
Standard #: VENDSUB-000-012-F Description: Based on the inspector's review of all four staff records, two of the staff members who have been employed for more than 90 days and do not have CPR and First Aid training. Evidence: Staff member #3 with a documented start date of 9/12/16 and #4 with a documented start date of 5/3/16 and have been employed for more than 90 days and do not have documentation of the completion of First Aid and CPR training. Plan of Correction: The administrator for the program will ensure both staff members are signed up for the next CPR/FA training to take place within five weeks of today's inspection.
