Technical Assistance:
Discussed documentation of references for staff. Discussed authorization period for long term medication and updating of children's files yearly.

Comments:
An unannounced inspection has been conducted from 10:00- 1145 AM. The Inspector reviewed classes, checked ratios, reviewed records, and special care provisions. The center has kept current on inspections, evacuation drills, and attendance records. The staff have attended various trainings and files reviewed revealed all have had Orientation training. Standard #: 22VAC40-185-240-D-4
Description: Based on record review, medication review, and staff interviews, the center has not ensured that children for whom emergency medications have been prescribed shall always be in the care of a staff member who meets the requirements of MAT certification. Evidence: 1. Inspector requested information on children with medication issues and staff who are MAT certified. Staff interviews and record review revealed two staff have MAT certification but both staff are not present today. 2. Two children are present that may require emergency medication such as albuterol and/or epipen.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center has a floor in the preschool area that has chipping paint and a room that does not have a lock to prevent children's access which is still being worked on.

Plan of Correction: We are planning to out in a new floor but need 3 days to accomplish.

Standard #: 22VAC40-185-350-A
Description: Based on observation and staff interviews, one staff was not counted in the staff-to-child ratios for 10 minutes as they were observed cleaning the classroom while the other 2 staff were supervising the group of children outside.

Plan of Correction: We have a number of staff who are able to assist and will retrain staff on transition.

Standard #: 22VAC40-191-40-D-1-A
Description: Based on record review of three staff hired in the last 4 months, the center has failed to obtain either Criminal History record checks and Search of Central registry. Evidence: 1. Staff 1 was hired January 11, 2017 and the Criminal History search was not the correct search so does not meet the requirements. This is also the case for staff 2 and 3 who were hired over 30 days ago. 2. Staff # 3 was hired on February 22, 2017 and also does not have a CPS clearance.

Plan of Correction: We will find the correct form, complete it online and send.
