Comments:
An unannounced monitoring inspection was made on 4/28/17 to determine compliance with Child Day Center standards that were not determined at the initial inspection. The inspector arrived at the center at 9:15 a. m. and the inspection concluded at 11:15 a. m. The following staff-to-child ratios and activities were observed: Infant Room: There was one infant with one staff. The infant was observed napping in his crib. Later, the infant was observed awake and had just completed an art activity with the staff. Two Year Room: There was one child with one staff. The child was observed making and decorating a puppet. Four Year Room: There was one child with one staff. The child was working at the computer station, playing a hide and seek game. Five staff records were reviewed. One child record was reviewed. The vehicle was checked. Conversations were held with all staff and two children. Classroom observations were made in three classes. No children were on any medications. Several staff are trained in MAT (medication administration trained), First Aid and CPR and DHO (daily health observation). The last monthly evacuation drill was completed on 4/20/17.

Standard #: 22VAC40-185-160-A
Description: Based on review of staff records the center did not have a current TB test on one staff. Evidence: On 4/28/17, staff #1 (whose start date was 3/20/17) had a TB Test dated 8/31/15. TB tests are not transferable if they are more than one year old.

Plan of Correction: Will get TB test updated today.

Standard #: 22VAC40-185-580-B
Description: Based on observation the center vehicle did not have a state inspection tag. Evidence: On 4/28/17, the new vehicle did not have a Virginia inspection sticker as required by DMV.

Plan of Correction: Will take the vehicle to the service station today.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff records the center did not have all required paperwork on file for one staff. Evidence: On 4/28/17, staff #1 (start date 3/20/17) did not have the central registry finding within 30 days of employment.

Plan of Correction: Will call today to follow-up.
