Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to have all areas indoors and outdoors maintained in a safe and operable condition. Evidence: a shelving unit housing art supplies in the downstairs classroom is in disrepair and leaning far to one side and could potentially fall. A post on the chain linked fence on the playground has a hole that has rusted, creating sharp points.

Plan of Correction: The shelving unit in the downstairs classroom will be removed. The post on the fence was temporarily covered with duct tape to cover the sharp points.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the facility failed to have adequate resilient surfacing under all equipment with moving parts or climbing apparatus. Evidence: the Little Tikes climbing equipment on the playground was over bare grass and no longer had mulch under the climber.

Plan of Correction: Volunteers are coming next Wednesday to refurbish the resilient surfacing on the playground.

Standard #: 22VAC40-185-540-E
Description: Based on review of the first aid kit, the facility failed to have all emergency supplies. Evidence: the battery operated radio was not operational because the batteries were dead.

Plan of Correction: Center owner went to the the store during today's inspection and bought batteries for the radio.
