Licensing inspector arrived at 9:40 am and departed at 12:30pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there was eighteen children in care with 4 staff . Additional staff were present to assist in the classrooms. Children were observed playing outside, singing songs and tummy time. The sample size consisted of 4 children records and 9 staff records. Areas of non-compliance was reviewed with the director.

Standard #: VENDSUB-000-005-B
Description: Based on record review and interview, the vendor did not ensure for each group of children, the vendor maintained a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: There was not documentation of of departure of each child in care. Confirmed with staff.

Plan of Correction: The owner/program director will create a new form to include time of arrival and departure.

Standard #: VENDSUB-000-012-B
Description: Based on record review, the vendor did not ensure within the first 90 days of employment or subsidy vendor approval; all staff shall complete Virginia Preservice Training for Child Care Staff. Evidence : Staff #2 did not have documentation of Preservice Training.

Plan of Correction: Staff #2 is trying to retrieve the training documentation.

Standard #: VENDSUB-000-012-E
Description: Based on interview and 2 of 9 records reviewed, the vendor did not ensure staff providing care to children had CPR and First Aid within 30 days of the date of employment or 90 days from subsidy vendor approval. Evidence: During the subsidy inspection conducted on May 17, 2017, staff #6 ( Date of hire 09/11/2015) and staff #8 ( Date of hire 05/12/2008) did not have CPR and First aid training with in 90 days from subsidy vendor approval.

Plan of Correction: The owner/program director is confirming with CPR and First Aid instructor for a date for the class.

Standard #: VENDSUB-000-031-B-1
Description: Based on and interview and review of procedures, the vendor did not have emergency preparedness plan for lock down and reunification for evacuation, shelter in place and lockdown. Evidence: During the subsidy inspection conducted on May 17, 2017, the vendor did not hve lockdown procedures. The vendor did not have procedures for reuniting children with parents or authorized person designated by the parent to pick up the child for evacuation , shelter in place and lockdown.

Plan of Correction: The program director / owner will have emergency plan done in 30 days.
