Comments:
The licensing inspector conducted an unannounced monitoring inspection Wednesday, May 24, 2017 from 12:00pm to 2:50pm. The children were observed resting on cots for rest time. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Three children?s records and three employee records were reviewed during this inspection. Medication is being administered and was reviewed. A sample of four injury reports were reviewed at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 04/10/2017 Last shelter-in-place drills: 12/15/2016 Last fire inspection: 01/20/2017 Last health inspection: 05/02/2017 Total enrollment today: 48 Today, the following child to staff ratios were observed: Infants-4:1 Toddlers ?5:1 Preschool (two -five years old, rest time ratio) ?39:3 School Age ? not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 05/24/2017, the center did not ensure that one of five children's records reviewed did not contain a work phone number and place of employment of each parent who has custody. EVIDENCE: Child #3's record did not contain a work number for the place of employment listed for the child's father.

Plan of Correction: Will correct today at pick up

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records on 05/24/2017, the center did not ensure that two of the three staff records reviewed contained documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering. EVIDENCE: 1. The record of Staff #2, whose date of hire was 03/10/2017, contained documentation of one reference check. 2. The record of Staff #3, whose date of hire was 05/11/2017, contained documentation of one reference check.

Plan of Correction: Will fix it today.

Standard #: 22VAC40-185-280-B
Description: Based on observation on 05/24/2017, the center did not ensure that all hazardous substances such as cleaning materials were kept in a locked place. EVIDENCE: In an unlocked office across from the kitchen window cleaner, stainless steel cleaner and glass cleaner were observed in an unlocked cabinet.

Plan of Correction: Corrected on site. The items were placed in a locked cabinet in the kitchen.

Standard #: 22VAC40-191-60-C-1
Description: Based on a review of staff records on 05/24/2017, the center did not ensure that an original criminal history record report was received within 30 days of employment. EVIDENCE: The criminal history record report in the record of Staff #1, whose date of hire was 11/16/2016, is dated 04/28/2017.

Plan of Correction: Keep copies of the originals and document actions taken after that. Had sent documents off, but they were returned for corrections

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of staff records on 05/24/2017, the center did not ensure that a central registry finding was received within 30 days of employment. EVIDENCE: 1. The central registry finding in the record of Staff #1, whose date of hire was 11/16/2016, is dated 01/24/2017. 2. The record of Staff #2, whose date of hire was 03/10/2017, does not contain documentation of a central registry .

Plan of Correction: Keep copies of the originals and document actions taken after that. Had sent documents off, but they were returned for corrections
