A self-reported incident was received by the facility administration on 4/13/17. An inspection was completed and there was a violation resulting from the self reported incident. If you have questions or need further assistance please contact me. Thank you for your time and cooperation.

Standard #: 22VAC40-185-410-1
Description: Based on staff interviews and review of documents the facility failed to maintain compliance with this standard as required relating to physical punishment. Evidence: Based on staff interviews and review of related documentation staff #1 spanked and hit child #1 on the back of the head on 4/10/17. Staff #2 stated that she witnessed staff #1 spank child #1 on the bottom on 4/10/17; striking the child 3-5 times. Staff #3 stated that she witnessed staff #1 hit child #1 in the back of the head because the child pushed the staff member. Staff #4 stated that staff #1 told her on 4/10/17 that she had spanked child #1. Child #1 stated that he was spanked by staff #1 "but that it didn't hurt".

Plan of Correction: Staff was terminated immediately upon notification on this. The standard is reviewed with all staff during orientation to ensure compliance.
