- Technical Assistance:
The license cannot be issued until the final Certificate of Occupancy, Fire Permit, and proof of insurance are received by the Licensing Inspector. The center cannot open for care until the license is issued. It is recommended that the center use the CDC model forms, available online. It was discussed that shade must be provided on the playground during the summer months of June - August. It was reported to the Inspector that initially, children will bring their own lunch and snack foods from home.
An announced initial inspection for licensure was conducted today. The center is not yet open. This inspection was completed based on an inspection of the indoor and outdoor physical plant areas, and a review of administrative documents. This inspection was conducted between approximately 10:00 am and 12:30 pm. Thank you for meeting with me today. Standard #: 22VAC40-185-40-A Description: Based on a review of documents, it was determined that the center has not yet prepared all required administrative documents. Evidence: 1. There is no evidence of liability insurance. 2. There is no Playground Safety Plan. 3. There is no Injury Prevention Plan. Plan of Correction: All items will be completed before we open for care.
Standard #: 22VAC40-185-240-B Description: Based on a review of documents, it was determined that the center has not prepared all information that is necessary for staff. Evidence: There is no staff handbook. Plan of Correction: I will create a staff handbook before we open for care.
Standard #: 22VAC40-185-270-A Description: Based on observations, it was determined that the physical plant of the center is not ready to open and care for children. Evidence: On the playground, the following items must be addressed - the swings and climbing structure are rated for children age 5-12; the toddler play area is filled with debris and wood pieces; the swing hooks are rusted; placement nails are protruding out of the mulch enclosure pieces; there is not enough mulch in any of the play areas. In the interior of the building, the following items must be addressed - all construction materials must be removed; all classrooms shall be equipped with a variety of materials/toys/diapering necessities. Plan of Correction: The playground and all interior rooms will be completed and set up for use by children before we open for care.
Standard #: 22VAC40-185-420-A Description: Based on a review of documents, it was determined that the center has not prepared all information that is necessary for parents. Evidence: There is no parent handbook. Plan of Correction: I will create a parent handbook before we open for care.
Standard #: 22VAC40-185-540-A Description: Based on observations, it was determined that the center does not have all required emergency equipment. Evidence: There is no flashlight, no first aid kit, and no emergency radio on site. Plan of Correction: The emergency equipment will be on site before we open for care.
Standard #: 22VAC40-185-550-A Description: Based on a review of documents, it was determined that the center has not yet prepared all emergency information. Evidence: There is no complete emergency preparedness plan on site. Plan of Correction: I will complete the plan and train staff on the information before we open for care.
Standard #: 22VAC40-185-550-C Description: Based on a review of documents, it was determined that the evacuation maps are not complete. Evidence: The classroom evacuation maps do not include all evacuation routes, and shelter-in-place procedures. Plan of Correction: The maps will be completed before we open for care.
