- Technical Assistance:
A discussion was held with the PD regarding the required fall zones for preschool playground equipment (slide) on the fenced playground. The required fall zone is the height of the slide plus 4 feet for the front of the chute.
An unannounced monitoring inspection was conducted in the Center on June 13, 2017. Upon arrival in the center, the LI observed 52 children in care. The children's ages ranged from toddler to school-age. The children were separated in 5 groups according to their ages. The school-age group (14 children/2 staff) left the center midmorning on a field trip to participate in a swimming activity. The Program Director was on scheduled leave from the center however she came to the center during the inspection. The LI was assisted by the designated in charge staff member and the PD during the inspection. As reported by the PD, 85 children are currently enrolled in the program. The LI observed the children in each classroom. The children were engaged in self-initiated play and teacher directed activities. A group of children were observed completing a Father's Day craft. The children participated in free play on the outdoor playgrounds. Several groups were observed in the dining room during the lunch meal. After lunch, the children napped. A sample of 8 records, 4 children's and 4 staff members, was reviewed. Prescribed medications and an-0ver-the-counter medication were observed for 5 children. Documentation of the center's annual fire and health inspections was current. Documentation of the practiced emergency drills was reviewed. The on site inspection was conducted from 9:00am to 12:30pm. Findings of the inspection were reviewed with the PD at exit. Standard #: 22VAC40-185-510-C Description: Based on direct observations, the Center failed to follow their written procedures to prevent use of outdated medication. Evidence: The LI reviewed medications for 5 children. One of five prescribed medications to be used as needed had expired. The prescribed medication for child f. noted an expiration date of 04/2017 on the original box. Plan of Correction: The PD informed the child's parent upon notice. The expired medication will be returned to the parent and a current medication will be requested. Medications will be monitored on-going to ensure dates of expiration are current.
Standard #: 22VAC40-185-520-A Description: Based on direct observations, the Center kept an over-the-counter skin product beyond the expiration date of the product. Evidence: The LI reviewed over-the-counter sun screen products and diaper creams in the toddler room for two children. A skin product for child a. expired 02/17. Plan of Correction: The Teacher will return the product to the parent. Staff will monitor the dates of expiration on over-the-counter lotions and ointments on-going.
