Comments:
An unannounced renewal inspection was completed on June 19, 2017. The purpose of this visit was to measure the facility's compliance with most, but not all, of the licensing standards. The following ratios were observed: Infants-2:7; Toddlers-1:5; Twos-3:18; Preschool-2:12; Pre-K-2:14; School age-1:14.
The children throughout the facility were engaged in activity. The infants were observed playing on the floor, napping and swinging in swings. Children in the toddler through four year old classes were observed playing outside, playing on the "Fun Bus", singing songs, playing with toys, working in learning centers and working with manipulatives. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The facility was equipped with manipulatives and supplies and items were available to the children. The following information was reviewed: fire and shelter-in-place drills, liability insurance policy, parent agreements, and staff training information. Medication is administered; medication authorizations were reviewed. Staff is certified in cpr/first aid, daily health screening and MAT.

Violations:
Standard #: 22VAC40-185-90--A
Description: Based on a review of a sample of children's records, the facility did not ensure that a child's record contained required written agreements. EVIDENCE: The record of child #1 did not contain a statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; and a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.

Plan of Correction: We will print the agreements separately, for the children who do not have the new form in their records, to be signed by parents.

Description: Based on observations at the facility, the facility did not ensure that hazardous materials such as cleaning materials were kept in a locked place. EVIDENCE: The inspector observed an unlocked laundry room. Inside the laundry room, there was detergent on the floor and cleaning materials in a basket hanging on the trash can.

Plan of Correction: Corrected on site. Staff and parents will be reminded that doors must remained closed and locked at all times.
