Comments:
An unannounced monitoring inspection was conducted on July 17, 2017. The site director was present. Two groups of campers were inside doing activities. There were 12 children, 5th grade and up, with 1 staff; and 14 children, 1st and 2nd grade, with 2 staff. Outside there were 19 children, 3rd and 4th grade, with 2 staff; and 17 children, kindergarten, with 1 staff. Evacuation drills had been conducted. Four staff files and 6 children's files were reviewed. Inspection began: 9:50 Inspection ended: 12:30

Violations:
Standard #: 22VAC40-185-510-C
Description: Based on a review of medication authorizations, the center did not have a written authorization from a child's physician for a long term authorization. Evidence: Child # 6 had a long term prescription and the authorization had not been completed by the physician.

Plan of Correction: The parent will be requested to provide the documentation.
