Today an investigation into a self-reported incident was conducted in the Summer Camp program as a result of a child found unsupervised entering the building. After interviewing staff, reviewing the incident report and dismissal procedures/ daily count procedures, it was determined that the child was able to enter the building without staff unaccounted for and not within sight and sound supervision for approximately 5-6minutes. Standard #: 22VAC40-185-340-A Description: Based on interviews with Staff, it was determined staff failed to ensure care, protection and guidance while supervising children. Evidence: On Tuesday July 18, 2017, while outside playing Child #1 was able to exit the open play space located on the outskirts of the playground. At approximately 11:20am Child #1 was observed walking into the front entrance of the building near the front office. Staff #2, was in the front office and observed the child entering the building without a staff member. Child #1 stated that "he had to use the bathroom" Staff #3 took the child to the bathroom and then back to the outside play area. It was at this time that Staff #1 discovered that the child had left the outdoor play area and entered the building alone. Child #1 was left alone and unsupervised outside for at least 5-6 minutes without staff aware that the child was missing. Plan of Correction: Summer Camp director stated that they discussed with staff and children about making sure that they are always with a camp counselor and that permission is given when they would like to use bathroom. Director stated that staff was informed that they must actively scan the play area and supervise children at all times.
Standard #: 22VAC40-185-340-F Description: Based on interviews with Administrators and Staff, it was determined staff failed to provide sight and sound supervision for a child under 10 years of age. Evidence: On Tuesday July 18, 2017 at approximately 11:10-20pm Child #1 was found entering the front of the building without a staff member. The staff did not realize Child #1 (8 years old) had been able to exit the play area, and cross the outside parking lot unsupervised and enter the building. Child #1 was out of sight and sound supervision for at least 5-6minutes. Plan of Correction: Summer Camp director stated that they discussed with staff and children about making sure that they are always with a camp counselor and that permission is given when they would like to use bathroom. Director stated that staff was informed that they must actively scan the play area and supervise children at all times.
