Discussed proper supervision of classrooms, you must have another staff cover a classroom to maintain ratios. There was a lot of staff movement in and out of classrooms while Inspector was sitting in the office.
An unannounced renewal inspection was conducted from 10:00 am-12:15. At the time of entrance, 32 children were in care with 9 staff members present. Children were observed engaged in group activities and outdoor play time. A selection of staff and children records, and other documents were reviewed. There are no medications this date. Areas of non-compliance are identified in the violation notice. The violations were reviewed with the new center director. Standard #: 22VAC40-185-570-E Description: Based on observation, infant bottles were not dated. Evidence: There were 2 bottles found in the Crawlers room that were not dated. Plan of Correction: I will instruct the teachers to date the bottles.
Standard #: 22VAC40-191-60-C-1 Description: Based on record review, the center has not maintained an original criminal history record for each staff. Evidence: Staff #1, date of hire 4/24/17 does not have documentation of a criminal history record results by the end of staffs 30th day of employment. Plan of Correction: I will resubmit the criminal record request for this staff.
Standard #: 22VAC40-191-60-D-1 Description: Based on record review, there is no copy to show that the background checks were requested within 7 calendar days of employment. Evidence: Staff #2, date of hire 6/26/17, staff #3 date of hire 7/17/17 and staff # 4 date of hire 6/26/17 does not have documentation to show that the criminal history record was submitted for processing. The director failed to make copies of the reports prior to mailing them. Plan of Correction: I will make the copies of each report prior to mailing them.
