Comments:
An unannounced monitoring inspection was initiated today at 10:40am. The physical plant, 4 staff files, 4 children's files, annual fire and health inspections, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected today. There were 41 children under the supervision of 8 staff. During the inspection, children were observed playing outside on the playground area, participating in circle time, listening to a teacher read a book, singing songs and preparing for lunch. For lunch, the children were served chicken bites (or veggie nuggets), green beans, mixed fruit, bread and milk. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 6 staff certified in MAT (medication administration training), 3 staff certified in Emergency MAT, and 8 staff certified in MAT for epi-pens. Description: Based on file review, 1 out of 4 staff records does not have a TB test/screening completed within the past two years. Evidence 1. Staff #4's most recent TB test/screening was last completed on 6/10/15.

Plan of Correction: Staff #4 was to have this completed. We will ask her for this the next time she comes in to work.

Standard #: 22VAC40-191-40-D-3-D
Description: Based on file review, 1 out of 4 staff records does not have a criminal history record check (CRC) completed before three years since the last one was completed. Evidence 1. Staff #3's most recent CRC was completed on 7/14/14.

Plan of Correction: This was mailed out to be completed on 7/17/17.
