Comments:
An unannounced monitoring inspection was conducted today with the director. Fifty five children were present with seven staff providing supervision. The schoolagers were engaged in electronics, stations and tabletop activities while most of the preschoolers were resting for nap. Five children and staff files were reviewed today. One child had medication on site. Please submit your plan of correction by 8/2/17. Arrival time: 1:30 p.m. Departure time: 4:50 p.m.

Description: Based on observation, it was determined that the diapering pad in the infant A room was not nonabsorbent. Evidence: The diaper pad in the infant A room was ripped, therefore it was an absorbent surface for diapering.

Standard #: 22VAC40-185-570-A
Description: Based on observation, it was determined that the protective belt on a high chair was not fastened while in use. Evidence: An infant was being fed while placed in a high chair, however the protective belt was not fastened.
