An unannounced Subsidy Health and Safety Inspection was conducted on July 27, 2017 from approximately 10:10 a.m. until 2:35 p.m Upon entrance there were 34 children and 6 staff present with children. There were additional staff on site for administrative and program purposes. The record review sample consisted of 5 children's records and 7 staff records. Children were observed working on counting projects and bible stories.
Standard #: VENDSUB-000-006-B-11 Description: Based on observation and interviews, the vendor did not ensure there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection on July 27, 2017, there was one child (child #2) who has a diagnosed food allergies (nuts,eggs, and fish) and there was not documentation to include a physician's information on how to address an allergic reaction. Plan of Correction: The vendor will get a plan from the doctor.
Standard #: VENDSUB-000-012-B Description: Based on observation and interviews, the vendor did not ensure all staff providing care to children had obtained within the first 90 days of employment or subsidy vendor approval Virginia Preservice Training for Child Care Staff. Evidence: During the inspection on July 27, 2017, there was not documentation that staff #1, (date of hire October 1996) staff #2 (date of hire August 1995), staff #3 (date of hire June 26, 2017), staff #4 (date of hire 2004), staff #5(date of hire March 2000), staff #6 (date of hire August 1995 and staff #7 (date of hire August 25, 2016) had obtained the required Pre-Service training. The month and year of hire was confirmed by the center director. Plan of Correction: The vendor will have staff take training.
Standard #: VENDSUB-000-031-B-1 Description: Based on observation and interviews, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during an emergency to include evacuation, shelter-in-place and lockdown emergencies. Evidence: During the inspection on July 27, 2017, there were not any reunification plans documented in the emergency preparedness plan. Plan of Correction: The vendor will update the plan.
