Technical Assistance:
There are two areas of exposed metal on the both outdoor playground climbers. The rubber covering has worn away leaving the metal exposed. If not addressed, the metal may become rusty and possibly sharp. Standard 570.I was reviewed: A one-day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care shall be maintained at the center. - The center should ensure that the emergency formula on hand is tolerated by the formula fed infants in care. The new sworn statement or affirmation and CPS requirements reviewed.

Comments:
An unannounced monitoring inspection was conducted on 07/28/2017 from 10:05am - 1:55pm. At the time of the tour, there were 59 children, ages infant - school age, in care with eight staff. Children were observed during morning program time which included outdoor play. Lunch and nap was also reviewed. Records reviewed for eight children and six staff. The inspector additionally reviewed: medication, emergency supplies, written emergency plans, hand washing & diapering, documentation of children's injuries and staff training/qualifications. The violations are listed on the violation notice and were reviewed with administrative staff at the conclusion of the inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center shall be maintained in a safe operation. Evidence: 1. There is an area of peeling paint on the closet door in the school age classroom. 2. There is an area of peeling paint on the second step on the preschool outdoor climber. 3. The metal chain link fence, located between the preschool and school age playground, has an area of loose fencing at the bottom near the ground. These loose areas are sharp. 4. There is an area of loose and broken metal at the bottom of the covering around the bottom of the school age exit door frame. This loose area has areas that are sharp.

Plan of Correction: Work orders were placed on 7/28/17. Center Director will follow up with facilities for repairs for completion.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center failed to ensure that the diapering surface shall be used only for diapering or cleaning children. Evidence: 1. A staff person, in the two-year old class, was observed working on a dramatic play display at the diaper changing table. a. Letters of the alphabet were being cut out of butcher paper using the diapering table as a work space.

Plan of Correction: Staff was retrained on 7/28/17. Center management will monitor.

Description: Based on record review, the center failed to ensure that the written record of children's minor injuries shall include staff and parent signatures or two staff signatures. Evidence: 1. An injury report, dated 7/27/2017, lacked a staff and parent signature or two signatures.

Plan of Correction: Staff have been retrained on 7/28/17. Center management will monitor daily to assure incident reports are completed and signed properly each day.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center failed to ensure that tables shall be sanitized after each use for feeding. Evidence: 1. One table in the school age classroom had two drops of syrup and two sticky areas of dried syrup when viewed at 10:30am. a. Pancakes had been served earlier that morning for breakfast.

Plan of Correction: Staff have been retrained on 7/28/17 on center classroom cleanliness.
