An unannounced subsidy health and safety inspection was completed on July 25, 2017. The purpose of this visit was to measure compliance with some but not all, of the subsidy requirements. There were 30 children, 18 months to school age, in care at the time of the inspection. There were 4 staff members that were working with the children in their separate groups. All groups were within the proper ratio. Children in the toddler through preschool groups had just finished lunch and were preparing for nap. The school age children were eating a lunch of chicken nuggets, corn, fruit cocktail, graham crackers and milk. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The following information was reviewed: fire and shelter-in-place drills, liability insurance policy, parent agreements, and staff training information.

Standard #: VENDSUB-000-004-F Description: Based on observations at the facility, the facility did not ensure that a current list of all children's allergies, sensitivities, and dietary restrictions was posted. EVIDENCE: The food allergy of child #1 was not posted. Plan of Correction: We will train staff on the appropriate steps to utilize with children with food allergies. We will also post a picture of the child to give staff a visual.
Standard #: VENDSUB-000-005-B Description: Based on a review of documentation at the facility, the facility did not ensure that arrival and departure times were tracked for each group of children. EVIDENCE: The attendance did not document the arrival times of each child in each group. Plan of Correction: We will have a staff meeting to educate staff on the new requirements to prevent from happening again.
Standard #: VENDSUB-000-006-B-11 Description: Based on a review of a child's record, the facility did not ensure that a child with a food allergy had a written allergy plan that includes instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. EVIDENCE: The record of child#1, start date 6/28/16, did not contain a written allergy care plan. Plan of Correction: We will meet with the parent to let her know that the Center needs an action plan completed by the physician to assist in the event her child has a reaction to a food she is allergic to.
Standard #: VENDSUB-000-012-C Description: Based on a review of a sample of staff records, the facility did not ensure that all staff completed the Virginia Preservice Training within 90 days of the effective date of the vendor agreement. EVIDENCE: The record of staff person #1, start date 9/2015, did not contain verification of completion of the Virginia Pre-service Training. Plan of Correction: I will make sure that the preservice training is completed for all staff who have not completed it.
