Proposed program changes and license modification were discussed.
An unannounced monitoring inspection was conducted on August 8, 2017, between the hours of approximately 10:39 a.m. and 2:00 p.m. Thirty-three (33) children were enrolled and in the care of five staff members during the on site inspection. Five children's records and five staff members' records were reviewed. No medication was on site/administered, on this date. Posted information, safety equipment (first aid kit, flashlight and radio) and available documents were observed/reviewed. The snack provided on this date was observed to comply with the United States Department of Agriculture (USDA) Child and Adult Care Food Program requirements and included orange slices, hummus, crackers and water. Lunches were provided by the children's parents. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Observation of the summer camp program was completed. Activities observed on this date included: snack; science experiment; nature walk; songs; restroom, diapering and handwashing procedures; lunch; and departure of children. Staff/child interactions, behavior guidance and daily routines/transitions were observed. Staff members were observed to be positive, patient and supportive in their interactions with the children in their care. Substantial compliance was maintained with the licensing standards; exceptions to compliance were noted on the accompanying violation notice. Thank you for your time and assistance. Standard #: 22VAC40-185-60-A Description: Based on review of five children's records and discussion with staff on August 8, 2017, the program failed to obtain the addresses of two designated people to call in an emergency if a parent cannot be reached for two enrolled children. Evidence: 1. Review of child A's and child B's records, on this date, revealed that the emergency contacts did not include the designated persons addresses. 2. Staff #2 verified the lack of emergency contact addresses in the children's records. Plan of Correction: The children's parent will be contacted and the emergency contact addresses will be obtained.
Standard #: 22VAC40-185-70-A Description: Based on review of five staff records and discussion with staff on August 8, 2017, the program failed to obtain complete information/documentation for one employee. Evidence: 1. Review of staff #5's record, on this date, determined that verification of age, verification of reference checks and documentation of orientation training were not included in the record (date of employment - August 7, 2017). 2. Staff #2 confirmed the lack of documentation in the staff member's record. Plan of Correction: The required information will be obtained and included in the staff member's record.
