Comments:
A subsidy health and safety inspection was conducted on 8/23/17 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice. An unannounced monitoring inspection was completed on 8/23/17. There were 97 children present in seven groupings. Ten children's records and nine newly hired staff records were reviewed. Three emergency/as needed medications were observed and one daily medication was observed. The inspector arrived at the center at 7:15 am and departed at 12:30 pm. The children were observed during arrivals, during free time in the gym, during worship time, during morning snack, during free play times in classrooms, and during outside time. Several groups of children were also observed participating in Melody Makers.

Violations:
Standard #: VENDSUB-000-004-F
Description: Based on observation, the center failed to ensure that all allergies, sensitivities, and dietary restrictions were posted in a way that was immediately accessible to staff. Evidence: 1. There was a four year old child in care that had a sensitivity to a food. This sensitivity was not posted at the facility. The child was enrolled 8/7/17 and had written authorization from the parent in their record to post the sensitivity. 2. There was a school-age child who was not in care during the inspection but was in care for after school care. The child had a food allergy and it was not posted at the facility. The allergy action plan was signed for this child by the physician June 2017.

Plan of Correction: This was corrected during the inspection. Both were posted. The director will ensure that staff post any allergies, dietary restrictions, or sensitivities when the child is enrolled.
