The outdoor playground was inspected with no hazards found; due to previous rain an accurate measurement of the resilient surfacing could not be obtained. The center had the required postings displayed upon entering the building and in children's classrooms. Ten children's records and four staff records were inspected with the majority of the required information found, see attached violation notice. The following was also inspected: medication authorizations/ log, MAT certifications, parent agreements, injury prevention plan, playground safety procedures, emergency preparedness plan, and other pertinent information. Standard #: 22VAC40-185-140-A Description: Based on the inspection of children's records, the required physical examination was not present to review. Evidence: During the inspection of ten children's records, child #1 did not have a physical examination present in the record. Licensing standards require physical examinations to be obtained before the child's attendance or within one month after attendance. Plan of Correction: Per administrator: I will have the parent to obtain a copy of the physical so we can place it in the child's record.
Standard #: 22VAC40-185-550-B Description: Based on the inspection of the emergency preparedness plan the required procedural components were not listed. Evidence: During the inspection of the center's emergency preparedness plan, it was discovered the plan did not include procedures for securing special healthcare supplies under the evacuation and shelter-in-place sections. Plan of Correction: Corrected on site
