A Subsidy Health and Safety Inspection was conducted on September 19, 2017 to review supplemental health and safety requirements. Licensing Inspector arrived at 3:00pm and departed at 4:45pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 14 school age children present in care with two staff. Children were observed playing various tabletop activities in the cafeteria and having snack. The sample size was two staff records and five children's records. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-006-B-11 Description: Based on review, the vendor did not ensure that there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: After reviewing the record for Child #2, it was determined that the child had diagnosed food allergies and there was not a written allergy care plan. Plan of Correction: Person in charge will follow up with the child's parent regarding obtaining the allergy plan.
Standard #: VENDSUB-000-006-B-3 Description: Based on record review, in five of five children's records reviewed, the vendor did not ensure that all required information was maintained in each child's record. Evidence: During the inspection conducted on September 19, 2017, all five children's records reviewed did not include the parents employment addresses. Plan of Correction: Person in charge will ask the parents to personally write their place of business (addresses) on the registration forms. I will follow up with the coordinator to change the registration forms.
