Technical Assistance:
Consultation was provided to center management on the following: 1. Background check requirements 2. Crib sheets fitting tightly 3. Posting children's information in a confidential manner 4. Model forms for medications

Comments:
An unannounced renewal inspection was conducted from 9:40 am-1:30pm. At the time of entrance, 97children were in care with 22 staff members present. Children were observed engaged in group activities and outdoor play time. Infants were observed being fed and sleeping. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, a child's record did not have all required authorizations. Evidence: The authorization emergency medical care, report of communicable diseases and sick child pick up policy was not authorized by the parent for Child # 5.

Plan of Correction: The forms will be signed by the parent today.

Standard #: 22VAC40-185-270-A
Description: Based on observation, an areas, inside the center was not maintained in a clean, safe and operable condition. Evidence: There was a hole in the wallboard behind the toilet, exposing insulation.

Plan of Correction: The hole will be repaired.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children. Evidence: A bottle of disinfectant spray was stored on a shelf that did not prevent access by children.

Plan of Correction: The bottle was removed during the inspection.

Standard #: 22VAC40-185-550-F
Description: Based on observation, a 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center was not posted in a visible place at each telephone. Evidence: The required emergency numbers were not posted by the telephone in the office and a classroom.

Plan of Correction: Numbers are being posted today.

Standard #: 22VAC40-185-560-G
Description: Based on observation, infant milk bottles were not clearly dated. Evidence: Milk bottles for Child # 2 and Child # 3 were not dated.

Plan of Correction: The dates will be added today and parents will be reminded.

Standard #: 22VAC40-191-60-B
Description: Based on record review, newly hired staff did not have the most current Sworn Disclosure Statement (SDS). Evidence: 1. Staff # 1 was hired 9/25/17 and staff # 2 was hired 9/18/17. Neither staff completed the required SDS that went into effect July 1, 2017.

Plan of Correction: Staff will complete the new SDS today.

Standard #: 63.2-1720.1-B-3
Description: Based on record review, child day center must obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him. Evidence: 1. Center did not have a system in place that showed that child protective searches were conducted for current staff (21 staff) that may have resided outside the state of Virginia in the last five years.

Plan of Correction: Center management had staff on site complete the new SDS form. Staff that has lived outside VA within the past 5 years will submit the required background checks today.
