An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 12:50PM and departed at 3:10 PM. Twenty children were in care with eight staff. Nap time and afternoon activities were observed. The playground was inspected. Medication administration and injury records were reviewed. A sample size of five children's records and five staff records (to include one new employee since the previous inspection) were reviewed. An exit meeting was condcuted with the program director prior to closure of the inspection.
Standard #: 22VAC40-185-280-B Description: Based on observation, the facility has not ensured that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: A high cabinet in the back adult bathroom in the preschool cabinet was unlocked. There were two containers of hand soap refills labeled "caution, keep out of reach of children". Plan of Correction: The facility responded with the following: All staff will be reminded that hazardous substances must be kept in cabinets with child proofing locks.
Standard #: 22VAC40-185-550-L Description: Based upon review of written injury records, the facility has not ensured that parents are notified by the end of the day of any known minor injuries. Evidence: The facility had written documentation of a minor injury to child 6 that occurred on 8/2/2017. The documentation was signed by the parent on 8/4/2017. There was no other documentation indicating that the parent was notified on the same day of the injury, 8/2/2017. Plan of Correction: The facility responded with the following: All family service staff will be instructed to add the parent telephone contact documentation directly onto the injury record. The person in charge to close the center will be responsible for ensuring that all accident incident forms received after the health coordinator leaves includes documentation that the parent was contacted prior to the end of the day.
