Comments:
The licensing inspector conducted an unannounced monitoring inspection that began at the center at 10:25am and concluded with staff record reviews at the center office at 1:00pm. There were fourteen preschool children in care with three staff. The children were observed finishing morning snacks, playing in the centers and playing with play dough. One staff and five children's records were reviewed. Areas of non-compliance are identified on the violation notice. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain outside play equipment in a safe condition. Evidence: 1. The inspector observed the upper playground. Two ground supports are uncovered on the far side of the bouncing air plane. One bolt for the ground support is bent and creating a tripping hazard. The other bolt for the ground support is protruding and could entangle clothing or snag skin. The grill on the front of the play bus is rusty.

Plan of Correction: A work order will be completed today for maintenance on the equipment. The children will not use this equipment until maintenance is completed.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement, the center failed to have 6 inches of resilient surfacing in areas on the playground. Evidence: 1. The inspector observed the lower playground and measured the resilient surfacing. There was 2 -3 inches of resilient surfacing (wood chips) surrounding the green and red slides.

Plan of Correction: A work order will be completed today for additional wood chips to be added to these areas of the playground.
