Discussed the recent communication regarding fingerprint based background checks for child day programs. Anyone who has lived in another state in the past five years is required to request a check of the child abuse and neglect registry for each state they have lived in the past five years. New applicants and agents for licensure, approval, and registration must request an out of state check when they initially apply for a license, approval, or registration.
The licensing inspector conducted an unannounced monitoring inspection Wednesday September 27, 2017 from10:45am to 1:00pm. The children were observed in "open time". During open time the children are self directed and choose which learning classroom they would like to work in. They were observed balancing on seat spinners in the gross motor room, playing in the dramatic play room and playing outside on the playground. All classrooms and playgrounds were inspected today. Ten children?s records and ten employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 09/26/2017 Last shelter-in-place drills: 03/27/2017 Today, the following child to staff ratios were observed: Pink Room -10:2 Purple Room -13:2 Yellow Room -14:3 Red Room-10:1 Blue Room -12:2 Rainbow Room -13:3 Orange Room-9:2 Polka Dot Room-13:2 Green Room -8:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-160-A Description: Based on a review of staff records on 09/27/2017, the center did not ensure that two of ten records reviewed contained documentation of a negative tuberculosis (tb) screening within 12 months prior to or 21 days after employing. EVIDENCE: 1. The earliest documentation of a negative tb screening in the record of Staff #1, who started at the center on 07/01/2017, is dated 08/27/2015. 2. The earliest documentation of a negative tb screening in the record of Staff #7, who started at the center on 02/06/2017, is dated 09/07/2017. Plan of Correction: I will make sure that new hires have tb within 21 days of hire.
Standard #: 22VAC40-185-510-N Description: Based on a review of medication on 09/27/2017, the center did not ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center. EVIDENCE: The authorization for medication for Child #11 expired on 08/23/2017. The medication was located at the center. Plan of Correction: Called parent today to bring updated information by tomorrow.
Standard #: 22VAC40-191-60-C-1 Description: Based on a review of staff records on 09/27/2017, the center did not ensure that two of ten record reviewed contained an original criminal history record report within 30 days of employment. EVIDENCE: 1. The criminal history record report in the record of Staff #7, whose date of hire was 02/06/2017, is dated 04/03/2017. 1. The criminal history record report in the record of Staff #8, whose date of hire was 04/01/2017, is dated 06/21/2017. Plan of Correction: Will make sure to have back within 30 days.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of staff records on 09/27/2017, the center did not ensure that two of ten record reviewed contained a central registry finding within 30 days of employment. EVIDENCE: 1. The record of Staff #2, whose date of hire was 08/01/2017, did not contain documentation of a central registry finding. 1. The central registry finding in the record of Staff #9, whose date of hire was 07/01/2016, is dated 02/02/2017. Plan of Correction: Will make sure to have back within 30 days.
