Today an unannounced monitoring visit was conducted with Director, this visit took place in the morning between 19:30am-12 there were 2 buildings observed with a total of 36 children with 4 staff within the supervision guidelines. The children were observed having morning snack, free play, learning time, counting, reading, sewing, lunch and in transition for dismissal. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-160-A Description: Based on review of the staff records, one record did not include documentation of a Tb screening within 12 moths prior to employment. Evidence: Staff #1's record with a hire date of 9/17, did not have an updated Tb screening two years from the date or the last screening or 12 months prior to employment (1/15). Plan of Correction: Director stated that the staff member just relocated and is in the process of looking for a new doctor. But will have it completed immediately.
Standard #: 22VAC40-185-70-A Description: Based on review of the staff records, one record did not include documentation of written orientation was done prior to first day of employment. Evidence: Staff #1, with a hire date of 9/17 did not have documentation of orientation training. Plan of Correction: Director stated that they are planning to meet with staff today to complete the orientation training.
Standard #: 22VAC40-185-280-H Description: Based on observation of the physical plant, medication and a purse was found unlocked and accessible to children. Evidence: In building #2, medication and a purse was found in a unlocked storage closet accessible to children. Plan of Correction: Staff immediately locked the closet during this inspection.
