Comments:
10/17/17 Arrived 9:55am Departed 1:25pm An unannounced renewal licensing inspection was conducted today. There are 82 children in care ranging in age from 2 1/2 years to 5 years with 11 teaching staff, a director and two office staff. The children were playing outdoors, reading books, eating lunch, having group time, singing and two classes were on a field trip. Please contact me if I may be of service 276 608 3749. Standard #: 63.2-1720.1-B-3
Description: Based on a review of five staff files, the facility failed to submit an out of state background check for all employees who have lived outside Virginia in the last five years. Evidence: 1. Staff #2, 3, 4, 5, 6 live outside Virginia and do not have a background checks from the state where they currently reside.

Plan of Correction: The program director reported that the facility was unaware of the new requirements. All employees have completed the background request and the director submitted the forms to the appropriate states.

Standard #: 63.2-1721.1-B-3
Description: Based on the review of two board officers background checks, the facility failed to obtain current out of state background checks.
Evidence:
1. Board officers 1 and 4 live in Tennessee and have not completed a background check for that state.

Plan of Correction: The program director reported that the
facility was unaware of the new
requirements. All board officers will complete the background request and
the director will submit the forms to the appropriate state.
